Here’s the thing nobody tells you when you set up your therapy practice: there’s a ceiling. A hard, unavoidable ceiling built into the one-to-one model. You have a fixed number of hours. You can only see so many clients. And when you stop working, the money stops too.

That’s not a you problem. That’s a business model problem, and building income beyond 1:1 hours is how you fix it.

Let’s say you charge £60 an hour and you see 20 clients a week. That’s £1,200. Not bad. But what happens when you’re sick? Or your kid needs you? Or you take a much-needed week off?

And here’s the bit that really stings: you could double your prices (which, by the way, you probably should), and you’d still hit the same ceiling. Just slightly higher up.

The one-to-one model is brilliant for so many things. But income beyond a certain point isn’t one of them.

What “Creating a Course” Actually Means for Therapists

When I talk about courses, I don’t mean filming yourself in a studio or becoming a full-time content creator. I mean packaging up what you already know into something people can access without you being in the room.

Think about it. Every week you explain the same things to different clients. You guide people through the same processes. You give the same advice in slightly different ways to different people who need it.

What if you recorded that once and sold it 100 times?

That’s the idea. A digital product, an online course, a self-paced programme. Call it what you like. The point is: you create it once, and it earns for you whether you’re in clinic, on the school run, or on holiday.

This is what I mean by building income beyond 1:1 hours.


“But I’m a Therapist, Not an Online Business Person”

I know. I hear this a lot.

And I want to say something clearly: you don’t need to become someone you’re not.

You don’t need to be a tech wizard. You don’t need to post on Instagram five times a day. You don’t need a big audience (though we’ll work on that too). You definitely don’t need to talk to camera like a YouTuber if that makes you want to hide under a blanket.

What you do need is to stop treating your expertise like it only counts when someone is lying on your treatment table.

Your knowledge is valuable. The transformation you create for clients is real. And it doesn’t have to be delivered in person to count.

Qualified isn’t the same as booked. And booked isn’t the same as properly paid.


What Gets in the Way (and Why It’s Not What You Think)

Most therapists who want to create a course get stuck at one of three points:

1. “I don’t know what to teach.”

You know more than you think. The trick is finding the thing that’s specific enough to be useful, broad enough to apply to lots of people, and packaged in a way that makes someone go “oh, that’s what I need.” We’ll work through this together.

2. “I wouldn’t know where to start with the tech.”

The tech is genuinely the smallest part of this. Once you know what you’re creating and who it’s for, the platform almost picks itself. Don’t let worry about tools stop you from starting.

3. “I don’t have an audience to sell to.”

This one’s real, and I won’t pretend otherwise. Building an audience matters. But you don’t need thousands of followers to sell a course. You need the right people, and a message that speaks directly to them.

I want to be honest with you: this isn’t passive income from day one. Nothing worth having is.

You start to feel like a business owner, not just a therapist who happens to charge.
